Should entries be in a particular format for attachment?

Trust me to get that wrong! I didnt put the email in the attachment! duh! Do you think it gets disqualified then? What an idiot am I!

I did the same, but I have also included my name within. I hope I did not do wrong. After all, the email address can very well contain my name.
