I believe the publisher (usually) decides which direction to take the cover, and I have heard that agents sometimes have to referee bunfights between writer and publisher (something along the lines of "Ta-daa!" - "Eh? That's horrid!").
I happen to have (suprise, surprise) quite strong feelings about the cover of my book! I believe it is also usual to have two covers, one for children and one for older teens.
At the end of the day however, the publisher knows best.
Funnily enough, I've done mine. I've spent the day teaching myself to use Gimp (GNU Image manipulation Programme) using Youtube tutorials. It's fairly basic but would be fine for self-publishing. Or it will be once I've taken the photo and added it to the background I've created. If I were to hand over to a publisher I would hope they'd at least look at it and include some ideas.
